Important Notice
All Companies Having Company/Agent or Agency Agreement with
Brooke Corporation
Brooke Corporation filed Chapter 11 with the U.S. Bankruptcy Court, District of Kansas, Kansas City, Kansas, on October 28, 2008. Any outstanding amounts of premiums, premium refunds, agent's commission, etc. with Brooke Corporation should be sent to the U.S. Bankruptcy Court.
A Proof of Claim needs to be filed. A hard copy can be obtained from any bankruptcy clerk's office (in Kansas--Wichita, Kansas City or Topeka).
The Proof of Claim form can also be obtained from the U.S. Court's Web site at http://www.uscourts.gov/rules/BK_Forms_Pending_2008/B_010_1208.pdf.
After the completion of the Proof of Claim, it needs to be filed at

Please be advised, premium paid by Kansas consumers to a Brooke Corporation agent/agency and/or the Special Master appointed by the court shall be treated as constructive receipt of premium. For other states, please contact the appropriate state insurance department.
No cancellation of coverage will take place if payment of premium has been made to any Brooke Corporation agent/agency.
