License Requirement

Completion of VL-1
$100 initial license fee
Completion of Company Appointment/Cancellation Form by Viatical Settlement Provider, if Applicant is a Viatical Representative
$50 renewal fee due March 1
Must be securities licensed if selling viaticated policies to investors

License Issued and Maintained

Upon processing the above material, a viatical license will be issued
The license is to be renewed annually by March 1
