Continuing Education Provider Forms
The forms listed below are used in administering Kansas continuing education requirements for agents. Please carefully review these forms, the administrative guidelines, and K.S.A. 40-4902 and K.S.A. 40-4903.
All forms are in DOC format unless otherwise indicated.

- NAIC Uniform Continuing Education Reciprocity Course Filing Form and Instructions, if applicable
Form KCE-2 must be completed by providers who submit and present courses. The nonrefundable $100 annual provider registration fee must accompany the initial submission of this form. The $100 fee will be required on an annual calendar year basis thereafter.
Form KCE-3 is intended for use by providers in submitting programs for approval in Kansas. This form should be submitted in DUPLICATE with the appropriate syllabus and/or text, plus the appropriate nonrefundable fee(s) indicated in K.S.A. 40-4903, payable to the Kansas Insurance Department ($50 per course filed or $250 annual fee payable with the first course submission of the calendar year). Upon review by the Department, one copy of this form will be returned to the provider indicating approval or disapproval along with an approval letter indicating the course ID# and the number of hours approved.
Form KCE-4 is for the purpose of providing evidence of program completion to the insurance agent. All information must be provided, and each form must be signed by the instructor or the provider’s authorized administrator. A facsimile signature is acceptable. The agent should maintain this certificate so that copies can be made and submitted to the Department. Institutional or association certificates are not to be issued for continuing education verification. The KCE-4 is the only acceptable course certificate. Incorrect/invalid information entered on the KCE-4 will result in a monetary penalty per occurrence.
All KCE-4's for the total credits required for the biennium must be submitted by each agent not later than their individual due date.
